( d) certificate of origin, one original issued or endorsed by the chamber of commerce of the country of origin.
( e) Ocean Bill of lading, three (3) original and three
(3) non-negotiable copies, full set, clean on board, issued to order of the buyers bank, blank endorsed and notify the applicant, marked freight prepaid, evidencing the L/ C number.
( f) beneficiarys certificate confirming copies of the shipping documents were sent by courier service to the following address:
